Ιδρύθηκε πριν το 1169, έτος κατά το οποίο μνημονεύεται για πρώτη φορά. Κατά δεύτερρο μισό του 14 αι., όταν ηγούμενος ήταν ο Χαρίτων ο Ίμβριος, η Μονή ενισχύθηκε πολύ και ανακαινίσθηκε με δαπάνες των μονών των παραδουνάβιων περιοχών. Αν και ο Χαρίτων έγινε μητροπολίτης Ουγγροβλαχίας το 1372, συνέχισε να ηγουμενεύει στο Κουτλουμούσι, ευεργετώντας το και ρυθμίζοντας τις σχέσεις των ελλήνων και των ρουμάνων πού μόναζαν στη Μονή. Το Καθολικόν, τιμώμενο στη Μεταμόρφωση του Σωτήρος, κτίσθηκε κατά το β΄ μισό του 14ου αι. Και τοιχογραφήθηκε έναν αιώνα αργότερα.

Ο νάρθηκας τοιχογραφήθηκε το 1744. Το παρεκκλήσι «της Φοβεράς Προστασίας», με την ομώνυμη θαυματουργό εικόνα, κτίσθηκε το 1733. Στην Μονή υπάγεται η παρακείμενη Σκήτη του Αγίου Παντελεήμονος (η λεγομένη και «Κουτλουμουσιανή») και το κελλί του Προδρόμου στις Καρυές, το ιδρυμένο από το Διονύσιο τον εκ Φουρνά.

The monastery was founded before 1169, when it is first mentioned in records. During the second half of the 14th century, when Chariton of Imbros was abbot, the monastery was generously endowed and restored by the princes of the lands along the Danube. Although Chariton became metropolitan of Hungro-Wallachia in 1372, he continued to hold the office of abbot at Koutloumousi endowing the monastery and regulating relations between its Greek and Romanian monks. The katholikon, dedicated to the Transfiguration, was built in the second half of the 14th century and was frescoed in 1744. The chapel of the Dreadfull Presentation housing the miraculous icon of the same name was built in 1733. The monastery has jurisdiction over the nearby skete of St Panteleimon known as Koutloumousiani, and the cell of Forerunner (Prodromou) at Karyes, founded by Dionysios of Fournas.
